Helpful checklist for your appointment or document upload.

• Photo ID (driver's license or state ID)
• Social Security card or ITIN for you, spouse, and dependents
• All W-2 forms
• All 1099 forms (NEC, MISC, INT, DIV, R, K, SSA, G)
• Last year's tax return (if available)
• 1098-T (tuition) and 1098-E (student loan interest)
• Mortgage interest 1098 and property tax records
• Childcare provider name, address, EIN, and amount paid
• Self-employed: income totals, expense receipts, mileage log
• Bank account & routing number for direct deposit
